RESUMO

Several studies have been published in 2023, in the field of general internal medicine, addressing various aspects of health and covering topics ranging from hormonal treatment of menopause to the management of chronic and acute diseases. A selection of articles was chosen for this update in outpatient general internal medicine, showing recent studies on outpatient care of patients (treatment, follow-up). The summary of articles is supplemented this year by a table of medical recommendations published in 2023, modestly proposed, and covering various areas. These guidelines, most of them from North America, show advances in the care and monitoring of outpatients, and should be interpreted in light of the various recommendations in Switzerland.


En 2023, plusieurs études ont été publiées dans le champ de la médecine interne générale, abordant divers aspects de la santé et couvrant des sujets allant du traitement hormonal de la ménopause à la gestion de maladies chroniques et aiguës. Un bouquet d'articles a été choisi pour cette mise à jour en médecine interne générale ambulatoire, traitant d'études récentes sur la prise en charge en ambulatoire (traitement, suivi) des patientes et patients. Le résumé d'articles est complété d'un tableau de nouvelles recommandations médicales de 2023, humblement proposé, couvrant divers domaines. Ces guidelines, pour la plupart américaines, montrent les avancées dans la prise en charge et le suivi des patientes et patients en ambulatoire, et sont à interpréter à la lumière des différentes recommandations en Suisse.

BACKGROUND: The early introduction of palliative care can have a positive impact on the quality of life of patients suffering from life-limiting diseases. However, the palliative care needs of older, frail, housebound patients are still mostly unknown, as is the impact of frailty on the importance of these needs. OBJECTIVES: To identify the palliative care needs of frail, older, housebound patients in the community. METHODS: We conducted a cross-sectional observational study. This study took place in a single primary care center and included patients who were ≥65 years old, housebound, followed by the Geriatric Community Unit of the Geneva University Hospitals. RESULTS: Seventy-one patients completed the study. Most patients were female (56.9%), and mean age (SD) was 81.1 (±7.9). The Edmonton Symptom Assessment Scale mean (SD) score was higher in frail patients as opposed to vulnerable patients for tiredness (p = 0.016), drowsiness (p = 0.0196), loss of appetite (p = 0.0124), and impaired feeling of well-being (p = 0.0132). There was no difference in spiritual well-being, measured by the spiritual scale subgroup of the Functional Assessment of the Chronic Illness Therapy-Spiritual Well-Being scale (FACIT-sp) between frail and vulnerable participants, although scores in both groups were low. Caregivers were mainly spouses (45%) and daughters (27.5%) with a mean (SD) age of 70.7 (±13.6). The overall carer-burden measured by the Mini-Zarit was low. SIGNIFICANCE OF RESULTS: Older, frail, housebound patients have specific needs that differ from non-frail patients and should guide future palliative care provision. How and when palliative care should be provided to this population remains to be determined.

The latest ESC recommendations propose several interesting new concepts for the practitioner. The recommendations distinguish between the «apparently healthy¼ patient and the patient at specific cardiovascular risk (diabetes, renal failure, and familial hypercholesterolemia). New risk calculation tools are proposed (SCORE2 and SCORE2-OP). The proposed LDL-C targets are specific to each group, as a general rule, < 1.8 mmol/l for individuals at high risk and < 1.4 mmol/l for individuals at very high risk. Presence of risk modifiers, comorbidities and patient preferences modulates therapeutic approach which is usually based on optimizing lifestyle and statin medication when necessary.


Les dernières recommandations de l'European Society of Cardiology (ESC) introduisent plusieurs nouveaux concepts intéressants pour le praticien. Le patient «en bonne santé apparente¼ est différencié de celui à risque spécifique (diabète, insuffisance rénale et hypercholestérolémie familiale). De nouveaux outils de calcul du risque cardiovasculaire sont proposés (SCORE2 et SCORE2-OP). Les cibles de LDL-C proposées sont spécifiques à chaque groupe avec, en règle générale, une valeur < 1,8 mmol/l pour les patients à haut risque et < 1,4 mmol/l pour ceux à très haut risque. La présence de modificateurs de risque, les comorbidités et les préférences du patient modulent l'approche thérapeutique, qui repose habituellement sur le respect des règles hygiénodiététiques et, au besoin, l'administration d'une statine.

Our selection of articles published in 2021 sheds light on topics related to risk, and to the use of electronic tools in primary care medicine. They cover blood pressure targets, telemonitoring, and the omega-3 fatty acid diet in the elderly and/or in patients with high cardiovascular risk. They present the role of primary care physicians in the management of patients with NAFLD, and in screening for domestic violence in all couples. They assess the risk of recurrence of a depressive episode after stopping antidepressant treatment. Finally, they discuss the place of apps to communicate with foreign-speaking patients and of vaping in smoking cessation.


Notre sélection d'articles parus en 2021 propose un éclairage sur des sujets en lien avec le risque chez nos patient·e·s et l'usage d'outils électroniques. Ils abordent le thème des cibles tensionnelles, du télémonitoring, et du régime riche en acides gras oméga 3 chez la personne âgée et/ou à haut risque cardiovasculaire. Ils présentent le rôle des médecins de premier recours dans la prise en charge de patient·e·s avec une stéatopathie métabolique, et dans le dépistage des violences conjugales dans tous les couples. Ils évaluent les risques de récidive d'épisode dépressif à l'arrêt d'un traitement antidépresseur. Enfin, ils discutent la place des applications pour la communication avec les patient·e·s allophones et du vapotage dans l'arrêt du tabac.

Integrative medicine proposes a coordinated approach to conventional medicine and complementary treatments. This approach is of particular interest in the field of prevention and in comprehensive primary care. This article presents some key studies published in 2020, which highlight the potential benefits of an integrative approach in primary care medicine.


La médecine intégrative permet de coordonner médecine conventionnelle et traitements complémentaires. Cette approche est particulièrement intéressante dans le domaine de la prévention et dans la prise en charge globale en médecine de premier recours. Cet article présente quelques études clés, publiées en 2020, qui mettent en lumière les bénéfices potentiels d'une approche intégrative en médecine de premier recours.

The scientific abstract is an essential part of an article. It resumes it clearly and synthetically in few sentences, while captivating the reader. Being the first paragraph, and sometimes the only one to be read, it should be comprehensive and needs to represent the paper as much as possible in order to motivate readers to continue reading if it corresponds to the subject of interest. It is also the only text to be submitted to conferences, and it is important in the process of application for research fundings too. In this article, we propose a practical guide to help you write a structured and accurate scientific abstract, with some practical tips.


L'abstract scientifique est une partie essentielle d'un article. Il doit le résumer en quelques lignes de façon claire et synthétique tout en captivant le lecteur. Etant la première partie de l'article, et parfois la seule partie lue, elle se doit de refléter au mieux le papier, afin que cette dernière soit lue en entier s'il correspond au sujet recherché. Il est également le seul texte soumis pour accéder aux congrès, et il représente un élément essentiel dans l'évaluation des dossiers pour obtenir des bourses ou des financements de recherche. Dans cet article, nous vous proposons un guide pratique pour vous aider à rédiger un abstract scientifique, afin qu'il soit structuré et précis.
